Transition Services
Transition planning starts when students turn 14 years old. Transition Goals are addressed on the student's IEP and are divided in to 11 categories. Hopp staff is available to answer questions and help with referrals or applying for outside services if needed.
The categories for Transition Goals are:
1. Vocational Training/Post Secondary Education: Students take part in vocational education at Syble Hopp School. They may participate in cafeteria jobs including set up, washing dishes, and serving food. Other jobs include maintenance, paper shredding, breakfast delivery, and lunch menu slip pick-ups.
2. Employment/Job Placement: Many high school students will be able to work in a community setting through the SHS Work Experience Program. Some students will qualify for Community Employment support after graduation while others will transition to an adult Work Service or Day Service Programs. All students and families will receive assistance with coordinating adult service programming before graduation.

3. Financial/Income Security: Information on funding for adult programming will be discussed. This includes applying for SSI, the Wisconsin Long Term Care Functional Screen/ Family Care, and the SSI “E” Supplement. Community resources and referral information can be provided.

4. Secure Living Arrangements: Types of residential support programs are discussed and information on requesting funding is provided.

5. Personal Management: Self advocacy, house hold and money management including Representative Payee services, personal care services and home health care, safety, communication, and daily living skills are just a few of the areas that can be covered.
6. Access to Leisure/Recreational Opportunities: Individual and family activities along with community recreational events and summer camps are included.

7. Transportation Services: Information on Medi- Van, Red Cross, Green Bay city bus transit, Lamer's transportation requests for after graduation, and driver's education for students with disabilities can be provided. Street crossing and pedestrian safety are also covered.
8. Medical Services: Medical coverage can come from private insurance or Medical Assistance. Information on different programs and how to apply is discussed.

9. Advocacy/Legal Services: This area covers guardianship and guardianship alternatives. It also covers community support services available to families.
10. Personal/Family Relationships: Encouraging friendships and appropriate social relationships are addressed. Families can also receive information on Respite and Family Support Programs.
11. Community Experiences: Syble Hopp curriculum has a strong focus on community experiences. Community settings are often our classrooms and may include accessing transportation, shopping, recreation, and restaurant experiences.
